Compose up to five custom questions to fit our database and enter them on the special "template" in the members area. When you enter them, they will also appear on your online survey, if you plan to link your site to this. See link you site for more details on how to do this.
The database is designed to allow you to use the fields for questions # 28 through #32. It is necessary that you use these numbers on both your printed and online survey, so that the questions and the answers in the database match.
These questions must be formatted to fit the database, which is designed for multiple choice questions. You may pose any question you wish, but each should be limited to 15 words.....and offer 5 multiple choice answers. Each answer should be no longer than 10 letters. Here is a good example:
Example: Which of the following best describes the service you normally get from our call center?
1. Excellent 2. Good 3. Adequate 4. Spotty 5. Poor
While it may seem limiting at first, crafting you questions to fit this model will make it easy and quick for your customers...and you will get much better response. Note: The template puts the questions into the database, your online questionnaire, and the upload data entry form. However, to put the custom questions into your card stock questionnaire, you must have your printer strip in the questions on the "Customizable Questionnaire", which you download from from the questionnaire download page

Beginning and Ending Date
You may change your custom questions as frequently as monthly. After you compose them, you will enter the beginning and ending month during which you want this particular set of questions in the database. During these months, you questions will appear on your upload template (data input) and your online survey. It is important to remember that you must input data into the correct months.
Composer for Custom Questions
After you sign up, you may enter the Member Area to compose your questions on the Custom Question Composer. After you compose them, they will automatically appear on your website (online) survey and your data upload templates.
