Survey Upload Template

After you receive your surveys back from our clients, you enter them into the FCSI database using out upload template. It is an easy-to-use, fill in the blanks template matching the questionnaire. Your work plan should allow for enough time for you to enter approximately 300 survey forms (or 25 per branch) into one of two special upload templates located in the Members Area. The online template allows you to enter data directly over the web into you database. The offline   template  is downloaded onto one or more of your local computers and will capture and store off line all of the input data for subsequent uploading.

With either template, all you need to do is enter the data. This is a keypunch function and will take approximately 1 to 2 minutes per survey. With 300 surveys to enter, you can plan on about 5 to 6  hours work. This could probably be spread over 3 or 4 days, if you are having an existing employee do it. Or you may prefer to bring in a student or temp to do this. Note: You will probably get many more surveys back than you need. It is not necessary to enter all of them. Follow the instructions in the Conducting the Survey section to reduce this to about 300 in total or 25 per branch.

Timing - The correct month!

As you enter the data be sure it is entered into the correct month....September surveys for September, etc. You control this  on the input template. If you are unable to enter all of your questionnaires by the end of the month, you may still  enter them using this menu. Your averages will change to reflect these late additions. While this feature gives you a "little cushion", it is still very desirable to plan to have all input done by month's end.

Once you get a feel for how many surveys you'll get back on a daily or weekly basis, you may have someone enter the data into the template each day or week rather than wait until the end.

Branches & Segments


The questionnaire itself asks customers to identify their branch, and this information should be entered directly into the template or database as you process the questionnaires.

Be sure we have your branches in the database. Once you have your user ID and password, you should go log in and select  Add/Modify Branches.  Use this same procedure when opening or closing branch locations.

Any "user defined" segments (e.g.- top deciles) will have to be defined by you and coded onto the questionnaires before they go out. That is what the little box at the bottom of the questionnaire is for. The only way to do this sort of segmentation sampling is via direct mail or phone, where you can identify the household and pre code the questionnaire. Also, be sure that your segments are put into the database via the Add/Modify Segments feature in the Members Area.
