You have three ways to enter hard copy data into the database (of course, you online survey will do this automatically):.1. On Line Upload Template (Partial Version Below). Your staff member will Log In to the member area and copy completed paper surveys to the on line version and submit survey data one by one. Note the Questionnaire ID box. This allows you to number each survey you enter in case you need to find it in the database! Nice for checking a clerk's work.

This is just part of the template. The "live" version has all questions, including your custom questions!2. Off Line Template. You may also download from the member area, a special template which allows you to enter your surveys offline in a batch mode, then upload all at once. This is especially useful if you have a low speed Internet connection or don't wish to remain online. The template is easy to use and fully described in the members area. It also allows you to save each month's or quarter's data on you computer (of course, we save it, too).
3. Send Survey to Us! If you prefer not to enter surveys at all, you may package them up and send them to us for data entry. While there is a fee, it may be a more economical solution for many.
