Choosing & Deploying Your Questionnaire
 

This website provides  you with all of the materials you need to conduct your survey. The hard copy questionnaire itself (linked below) can be down loaded with three different "front covers", depending upon how you wish to conduct your survey.

Some banks may wish to deploy the survey forms in their lobbies. Some may prefer to put them in the monthly statements. Others may prefer a direct mail letter with a note from a key executive.  Each method has its advantages and drawbacks which are discussed in our conducting the survey area.

Of importance here is the fact that we have designed a survey format for whichever method you prefer. And you may download a file of the questionnaire,  which you can take to a local printer (e.g. Kinko's) for reproduction. When you have it printed, be sure to specify a fairly firm "card stock" (e.g. #10).
 

The inside of the questionnaire (that is, the questions themselves) is the same, regardless of your method of deployment. As shown here, it is designed to make it quick and easy for the customer. You may customize the front as you wish, or use our front along with you logo.

You are given a choice of 3 "front covers", depending upon how you plan to deploy it.

Let's review these methods.

1.LOBBY   If you chose this method, select the Lobby Questionnaire. The cover of this one invites the customer to fill out and return the questionnaire to the branch, where you will have a Lobby Box available to collect them.

2.MAIL BACK-TO YOU   With this method you are inviting customers to fill it out and mail it back to you. You will need print your name and bulk mail return permit indicia on the back of the questionnaire. If you prefer this method, select Mail Back to You Questionnaire.

3.MAIL BACK-TO FCSI   If you prefer, you can opt to have us collect and tabulate the questionnaires for you. The Mail Back to Us Questionnaire has our tabulation service address preprinted. All you have to do is download put in your company’s name and print.

Customization of Questions (click here for guidelines for composition)

You may add up to 5 of your own questions to your survey by selecting one of our "customizable" options. (Please follow our Guidelines to compose your questions). The first uses the standard questionnaire interior, but provides space to put in one or two of your own questions. The second method allows you to download a MSWord version (see download options) of the questionnaire so you can add as many additional questions as you wish. In either case please do not change the standard questions (#1 thru #27 and the demographics) as this will negate your ability to compare results! If you use the MSWord version, you will need to provide a return envelope of your own.

Important: Please follow the Guidelines for composition. This will ensure that your custom questions are reflected in the database.

Don’t forget....you also have the on-line survey form, which we recommend, even if you don’t yet have your own web page. You can promote this to your web savvy customers via advertising, statement stuffers, monthly statements, etc.  See the  On-Line Questionnaire Summary  for suggestions on promoting,  and then view the actual questionnaire to see how easy it is to complete!
