Interior Format (the Questionnaire)

• FCSI Survey Standard Edition                                                                                                Standard  Interior

  The standard edition is in "camera ready" (.pdf) condition. There are 27 standard questions, plus 6 "Tell Us About Yourself" demographic questions. This is the easiest one to use. Simply download the survey from the download page, following the instructions there. And match it with your preferred Exterior (below).

  The survey is a 7"x7.5" single fold. It should be printed on "card stock" for return mail.

• FCSI Customizable Edition                                                                                                                                                      Customizable Interior

Provides additional white space for questions pertinent to your financial institution. Simply use this "customizable" interior with any of the three exteriors (below). Just "strip in" your questions where the blank space is following question # 27.  This survey will accommodate 3 to 5 additional questions.   See Customized Questions for more help.

• This is the most common exterior.

  It is designed so that customers may fill it out and mail it back to you. After downloading, put your organization's name and return address information where indicated. The download is larger and you can clearly see where to put in your information.

  This exterior format is designed for customers to complete and submit within your branch location.  Order a special FCSI Survey Collection Box
  from us or use your own receptacle. Take note of the additional white space available on this exterior format.  We suggest you use this space to your benefit.  Inform your customers of your on going effort to achieve their complete satisfaction with your organizations's operations. But don't print phone numbers or locations, as this may cause customers to keep it.

• MS Word Formatted Questionnaire
• You may also download an MS Word formatted questionnaire. This will give you even greater flexibility to add questions of your own.   This is not a "self mailer" though and you will have to provide your own envelopes. Please do not alter the 27 standard questions, however, as this will compromise your ability to benchmark your results. Also, please see Customized Questions for a description of how to compose up to 5 custom questions for the database.
