1. Create File Folder - For organization, create a file folder on your computer titled FCSI. We recommend you store all FCSI information in this folder. When prompted save your FCSI related files to this folder.
2. .PDF or .DOC - The questionnaires are available in either Adobe Acrobat format (.pdf) or Microsoft Word format (.doc). To view the files on your computer you must have one of these software programs loaded. To view .pdf format you must obtain the latest FREE copy of the Adobe Acrobat Reader by going to the Adobe Site and choosing the latest version for your computer type.
3. Download FREE FCSI Files - We suggest you save a copy of each of the formats listed below (three files). The format you elect to have printed on card stock will depend on how you choose to conduct and process your surveys.
4. Save Link As - Select the option "Save Link As" or "Save Target As" and choose to save to your FCSI file folder.
5. Take to Your Local Printer - Transfer your downloaded file(s) to a floppy disk or print a master of each version (you'll need a laser quality printer). Take the disk and master copies to your your local printer. Instruct the printer about identifying your financial institution, the font (see below) to use and weight of card stock.

Notice About Font and Typeface:
Preferred font is Helvetica* style when customizing your questionnaires. Preferred size is 10pt. However, if you add more than three "custom questions", you may have to use 8pt. type. Also, be sure to specify "card stock" paper.
*If Helvetica is not available, acceptable substitutes include Arial (most commonly available), Geneva, Futura, Universe, or Swiss.Codes and Identifiers*
You will notice that on all versions of the
questionnaire, there is a special box for coding. This allows you to generate reports by
individual user defined segments or other identifiers (e.g. MCIF Customer Types). You
will need to define and preprint your questionnaires with these codes, making sure that they
only go to those customers. Or insert codes manually by hand prior to a specific mailing.
